Failure to Return Application – This is identified through either the 2nd or 3rd & Final Notice on the reappointment and initial appointment processes. 3rd & Final Notice Letters, are sent to the MPR Team Assistant as a PDF. The MPR Team Assistant will transfer the letter and deactivation form into CM, e-mail the letter to the participating health care facility, and ‘cc the provider. The provider is deactivate in CM. The MPR Team Assistant will enter the termination date for the open activity and create a Failure to Return Application activity for the applicable facility(ies).
Withdrawn Application – This is identified when a client requests a file be withdrawn from the initial appointment/reappointment process. (Letters HAVE been sent.) The MPR Team Assistant will deactivate from both CM and the Data Bank. The notification, application, and processing documents will be scanned into CM. The MPR Team Assistant will enter the termination date for the open activity and create a Withdrawal of Application activity for the applicable facility(ies). The applicable participating health care facilities will be notified.
Decision not to Pursue/Non-Renewal – (BEFORE letters have been sent.) This is identified when the applicant has submitted an application but either the applicant or the client requests the application not be processed. The person signing this request needs to scan the file into CM and then mail or email the original application to the client (initial appointment only). The reason for the mailing is that the client may have already granted temporary privileges and may need this information as supporting documentation. There is no need to submit the file to final quality because no letters have been sent and there is no begin date in CM. The MPR Team Assistant will need to be notified because she will code the termination and perform the deactivation. The MPR Team Assistant will enter the termination date for the open activity and create a Decision not to Pursue or Non-Renewal activity for the applicable facility(ies).
Deactivation – No initial appointment or reappointment in process. This is identified when MPR has been advised by the client or provider to remove an active provider. The MPR Team Assistant will scan the notification into CM and deactivate from CM and the Data Bank. Notifications will be sent to the participating health care facility.
Termination from Group/Retirement – This is identified when MPR receives a notice from a group of a terminated or retired provider. The MPR Team Assistant will send the provider a letter to confirm retention or relinquishment of privileges and to acquire a forwarding address (‘cc facilities). The response will be scanned in to CM and all applicable participating facilities will be notified.
Death – This is identified when we are notified by the physician practice group or the participating health care facility. The MPR Team Assistant will make an entry into CM (either scan the letter or write an entry on the general tab) and notify the participating health care facilities. Deactivation from CM and the Data Bank are required.
