MPR Launches Initial Appointment Online Application!

medical provider resources initial appointment applicationMedical Provider Resources is beyond thrilled to announce after LOTS of work ensuring all aspects work correctly – our Initial Appointment Application is now online! Previously, applicants needed to complete a Request to Send Application (RSA) form, but now can easily just click over to our website on the green “Complete Application” button to sign in and fill out the form online.

In addition to being online and more efficient, here are a few things to note about the new Initial Appointment Online Application:

  1. Clients will continue to receive the Request for Consideration (RFC) email.
  2. Document update requests will be sent electronically to the credentialing contact on file at MPR.
  3. Reappointment applications will automatically be sent via email on the 1st of every month.
